Your first aligners: what happens at the fitting

At a glance

  • Before you go, we check that each aligner fits your teeth properly.
  • Some teeth may have small tooth-coloured “attachments” bonded to them. These help the aligners grip and move the teeth as planned, and are removed at the end of treatment.
  • Some plans include IPR (interproximal reduction), where a very small amount of enamel is polished from between certain teeth to create space. It is only done if your plan includes it.
  • We show you how to put your aligners in, take them out and clean them.
  • Aligners need to be worn for most of the day and night, usually 20 to 22 hours a day, or as your orthodontist advises. Take them out to eat, to drink anything other than plain water and to brush.
  • New aligners commonly feel tight for the first few days. You will move to each new set at the interval your orthodontist sets, with reviews along the way.

Summary

Your first aligner appointment is mostly about checking the fit and making sure you feel confident caring for your aligners at home. In this short video, Dr Sarah Sadek explains what happens, from checking each aligner and placing any attachments, to how long to wear them and what to expect in the first few days.

Clear aligners only work when worn as prescribed, so we also cover how to fit them into everyday life: removing them for meals, cleaning them and keeping them in their case when they are out. After treatment, retainers are needed to help hold teeth in their new position.

Consultations and aligner fittings take place at SW3 Dental, 295 King's Road, Chelsea.

Your questions answered

Are new aligners painful?

Most people feel some pressure or tightness for the first few days of each new set. This is normal and usually settles. If an edge feels sharp, or discomfort does not settle, contact the practice so we can check it.

How long each day should I wear aligners?

Usually 20 to 22 hours a day, removing them only to eat, drink anything other than plain water and clean your teeth. Your orthodontist will tell you what your treatment needs.

What are attachments, and do they stay on?

Attachments are small tooth-coloured bumps bonded to some teeth so the aligners can grip and move them. They are removed at the end of treatment and the surface is polished.

What does IPR mean?

Interproximal reduction (IPR) means polishing a very small amount of enamel from between certain teeth to make space. Not everyone needs it, and it is only done if it is part of your plan.
